What should a patient registration form include?
A good registration form for an Indian clinic answers four questions: who is this patient, how do we reach them and their family, what must the doctor know before examining them, and what did they agree to. The table lists each section, the fields and why you need them.

Collect only what you need. The Digital Personal Data Protection Act 2023 expects you to collect personal data for a stated purpose and no more than that purpose needs. Do not ask for Aadhaar unless you have a lawful need for it; a mobile number and date of birth identify most patients. Keep income, religion and caste off a clinic registration form.

How do patients fill the registration form online before the visit?
In Appointment Reminder App a form is attached to one or more services. From then on every booking of those services, whether made online, by staff, by CSV or through the API, gets a personal form link:
- Attach the form to a service. Start from the new patient registration template, edit the fields and attach it to services such as Doctor consultation.
- The link goes out with the booking. Each booking of that service gets a personal form link on the booking confirmation page and the appointment page.
- The patient unlocks it. The patient opens the link and enters the last 4 digits of their mobile number; labels show in their language with English below.
- The patient submits. Answers, an optional signature and a consent receipt are saved; the draft stays on the phone until submitted.
- The desk sees Form ✓. The day list shows Form ✓ or Form pending; staff print the answers for the file and copy mapped fields to the patient record.
Walk-ins use the desk link instead: put a tablet at reception or print the form's QR code. The patient enters name and mobile number first, and if they have an appointment today the answers are linked to it automatically. A submission locks when the visit starts; later changes by staff are recorded in the audit log.
Can the form be in Tamil or Hindi?
Yes. Each question can carry a Tamil and a Hindi label next to the English one. The patient sees their own language first with English underneath, for example "ஒவ்வாமை / Allergies", so the receptionist can still read every answer on the printout. The language follows the patient's reminder language, and the patient can switch on the form. Bilingual labels and signatures are on the Growth and Scale plans.

Field types: short and long text, number, date, phone, email, single and multiple choice, dropdown, yes/no with a details box, section headings, a consent tick and a signature drawn on the phone. Each answer can map to the patient record (name, date of birth, gender, address, ABHA number, emergency contact); "Apply to patient record" fills only empty fields unless staff choose to overwrite.
How is patient data protected?
Appointment Reminder App hosts data in India and encrypts it in transit. Each patient form link opens only after the last 4 digits of the patient's mobile number and a captcha, wrong attempts lock the link, and the page is never cached or indexed. Inside the app, roles decide who can read submissions, every amendment is in the audit log and each submission carries a consent receipt, as the DPDP Act 2023 expects. A signature is stored as an image; we make no claim about its legal validity. The forms are not an EMR and do not connect to ABDM. More on the security and compliance page.
