What is a patient no-show policy?
A patient no-show policy is a short, written set of rules that tells patients what counts as a missed appointment, how and when to cancel or change a booking, and what the clinic will do after repeated misses. It protects the slots that other patients are waiting for, and it makes the front desk's response predictable instead of personal.
A good policy is fair to patients who have genuine problems and firm with the few who book and never come. It is shown before booking, repeated in the confirmation and applied the same way to everyone. The policy wording below is a starting point, not legal advice; adapt it to your clinic and take advice if you are unsure.
Should Indian clinics charge a no-show fee?
Most Indian clinics do not charge a no-show fee, and many guides written for the United States assume something that rarely exists here: a credit card on file. Charging a patient after the fact means chasing money from someone who has already shown they are hard to reach. Three alternatives work better in India:
- A UPI advance after repeat no-shows. A patient who has missed twice pays a small advance, for example ₹200, by UPI link before their next online booking is confirmed. The advance is adjusted against the bill when they come. People who have paid tend to turn up, and patients who always come never see the rule.
- Desk-only booking. After repeated misses, online booking is politely refused and the patient is asked to call. The receptionist can then confirm the visit personally, the day before.
- A flag only. Staff see a badge next to the patient's name and call to confirm before the visit. Nothing changes for the patient.
When should you not enforce at all? Charitable clinics, government-linked programmes, camps and practices serving patients who depend on others for transport usually do better with reminders and a flag than with any restriction. Our playbook to reduce no-shows covers the reminders that should come first.
What should a no-show policy include?
Use this checklist before you publish your policy:
- Definitions: what is a no-show (did not come and did not tell us) and what is a late cancellation (cancelled less than a set number of hours before).
- Late-cancellation window: commonly 4 to 24 hours; shorter for walk-in-heavy clinics, longer for long procedures.
- How to cancel: reply on WhatsApp, tap the Cancel button, call a number, or use the link in the reminder.
- Consequences: what happens after how many misses within how many days (for example 2 in 60 days).
- Exceptions: medical emergencies, hospital admission, bereavement, and a lighter touch for senior citizens.
- Reset: when strikes expire (for example after 90 days, or after two attended visits).
- Contact: a phone number for questions and appeals.

[Clinic name]: Appointment and no-show policy
We keep time for every patient who books. To help us see everyone on time, please follow this policy.
1. Cancelling or changing an appointment. Please cancel or change your appointment at least [4] hours before the time. You can tap Cancel or Reschedule in our WhatsApp reminder, use the link in the message, or call [phone] between [9 am and 7 pm].
2. What counts as a no-show. If you do not come and do not tell us before the appointment time, it is recorded as a no-show. If you cancel less than [4] hours before, it is recorded as a late cancellation.
3. What happens after repeated no-shows. After [2] no-shows or late cancellations within [60] days, [online bookings will need an advance of ₹[200] by UPI, adjusted in your bill / please book by calling the clinic / we will call you to confirm future bookings].
4. Exceptions. We understand that emergencies happen. Missed appointments due to illness, hospital admission, a death in the family or similar reasons are not counted if you let us know. Senior citizens and first-time misses are treated with extra consideration.
5. Reset. No-shows older than [90] days are not counted, and the rule ends after [2] visits you attend.
6. Questions. If you think a no-show was recorded by mistake, please call [phone] and our front desk will review it.
Thank you for helping us care for every patient on time. — [Clinic name], [date]
Short version for your booking page and WhatsApp confirmation (English, Hindi, Tamil)
Use this short version (about 40 words) on the online booking page, with an "I have read the policy" tick, and as one line in the booking confirmation.
English
Please cancel or change your appointment at least 4 hours before. After 2 missed appointments without notice in 60 days, online bookings will need an advance of ₹200 by UPI. Emergencies are always excused. Call: [phone]
Which rule should you choose: flag, advance or desk-only?
Choose the lightest rule that solves your problem. Most clinics start with a flag, move to an advance for long or expensive slots, and keep desk-only booking for the few patients who repeatedly book and do not come.
| Flag only | UPI advance | Desk-only booking | |
|---|---|---|---|
| What the patient sees | Nothing | An advance request with the online booking | "Please call the clinic to book" |
| Staff work | A confirmation call before the visit | Almost none; the payment link goes out by itself | Bookings by phone for these patients |
| Best for | Most clinics, first offences, seniors | Dental, physiotherapy, diagnostics, long slots | Persistent no-shows |
| Risk | Low impact if the call is skipped | Patients without UPI need another way to pay | Can feel harsh; explain it kindly |
| Reversible | Yes | Yes, refund or adjust the advance | Yes, staff can forgive |
How to enforce it fairly
A policy is only fair if it is applied with judgement. Three habits keep it humane:
- Forgive with a reason. When a patient explains an emergency, clear the strike and note why. Keep a record so the decision is consistent across staff.
- Use a script. "We hold this time only for you, so we ask everyone to cancel at least 4 hours before. This time we have noted it; next time please reply Cancel on WhatsApp." Polite, firm and the same from every receptionist.
- Go easy on first offences. A single miss earns a reminder and a flag, never a restriction.
Sample message after a first no-show:
Hello [name], we missed you at your [time] appointment today. We hope all is well. Reply BOOK to choose a new time. Please let us know at least 4 hours ahead if you cannot come. - [Clinic name]
Sample message after a second no-show:
Hello [name], this is the second appointment missed without notice in 60 days. As per our policy, online bookings will need a ₹200 UPI advance for the next 90 days, adjusted in your bill. Emergencies are always excused: call [phone]. - [Clinic name]
Worked example: counting strikes
Illustrative example, not real patient data. A clinic's rule is "2 no-shows or late cancellations within 60 days → ₹200 UPI advance for online bookings; strikes expire after 90 days or 2 attended visits; late = cancelled less than 4 hours before".
- 3 March: Ravi books 10:30 and does not come. Strike 1. He is flagged; nothing changes for him.
- 20 March: he cancels at 9:00 for a 10:00 appointment. That is inside the 4-hour window, so it is a late cancellation. Strike 2 within 60 days: online bookings now need a ₹200 advance.
- 25 March: he books online, pays the ₹200 advance by UPI and attends. Attended visit 1.
- 15 April: he attends again. Attended visit 2: the rule ends.
- If he had stopped visiting instead, the rule would have ended on its own 90 days after 20 March.
Had Ravi phoned on 20 March to say his child was in hospital, the desk would forgive the strike with that reason, and he would stay at strike 1.
How to tell patients about the policy
Patients accept a policy they have seen before it affects them. Show it in three places: a printed poster at reception in English, Hindi and Tamil; the short version on your online booking page with an "I have read the policy" tick; and one line in the booking confirmation on WhatsApp. Never surprise a patient with a rule at the counter.
Appointment Reminder App applies this policy automatically. You set the late-cancellation window, the number of strikes and the days, and the action: flag, UPI advance or desk-only booking. Marking a no-show or a patient's late cancellation counts a strike; the online booking page shows the policy with a tick box and applies the rule by phone number; staff see a badge on the day list and can forgive with a reason, which is recorded in the audit log. A repeat no-show report shows which patients and which weekday and hour slots fail most often, and the app prints the policy poster in three languages. It does not charge cards or collect fees automatically.
See features: no-show policy, included in every plan from ₹2,999 a month plus GST on the pricing page. To estimate what missed visits cost you, try the no-show cost calculator, and see how the policy appears on the online booking page. For everything that should come before a policy, from reminders to waitlists, read the full playbook to reduce no-shows at your clinic; and set the policy up in minutes from the no-show policy settings.
Questions people ask
What is a no-show policy for a clinic?
A clinic no-show policy defines a missed appointment and a late cancellation, explains how to cancel in time and states what happens after repeated misses, such as a UPI advance or desk-only booking. Appointment Reminder App shows the policy at booking and counts strikes per patient automatically.
Can a clinic in India charge for a missed appointment?
Some clinics do, but card-on-file fees are uncommon in India, and collecting money after a no-show is hard. Most ask repeat no-shows for a small UPI advance before the next booking instead. This guide is not legal advice. Appointment Reminder App supports advances, flags and desk-only booking, not automatic fees.
How many no-shows before a patient is restricted?
Many clinics use two no-shows or late cancellations within 60 days, with a flag after the first and the rule after the second, and let strikes expire after about 90 days. In Appointment Reminder App you choose the number of strikes, the window in days and the reset period.
What is a reasonable late cancellation window?
Choose a window that leaves time to refill the slot: about 4 hours for busy walk-in clinics, 24 hours for long dental or physiotherapy appointments. Appointment Reminder App counts a cancellation as late only when the patient cancels inside your window, never when staff cancel.
How do I tell patients about my no-show policy?
Show it before it affects anyone: a reception poster, the short version on the online booking page with an "I have read the policy" tick and a line in the WhatsApp confirmation. Appointment Reminder App adds the tick to the booking page and prints the poster in English, Hindi and Tamil.
Should emergencies be exempt from the no-show policy?
Yes. Illness, hospital admission and bereavement should not count, and first-time misses deserve a reminder rather than a restriction. In Appointment Reminder App staff forgive a strike with one click and a reason, which is kept in the audit log, and the patient can book normally again.
